Don’t forget to complete and turn in your forms packet during the first week of school!

SCHNECKSVILLE, Pa. | The 2018-19 school year is here and that means it’s time for Lehigh Career & Technical Institute students to master new skills, make new friends…and complete their forms packet.

Every student must return a completed forms packet to their career and technical education program teacher during the first week of school. The forms must be on file in order for students to work in their program lab, use school computers, participate in field trips, be eligible for school-to-career opportunities and more. All of the forms require a parent or guardian signature.

The following items are included in the packet:

• Home Language Survey (for first-year students)
• Student/Parent Acceptable Use of Communications & Information Systems and Handbook Compliance Form
• Student Safety Pledge
• Food Allergy Permission Form
• Lunch Prepayment Form (for students who plan to buy lunch at school)
• Student Medical Card
